This form should be used when submitting a proposed amendment to an Operations Manual. Complete all sections and ensure the form is signed by the appropriate responsible managers. Failure to complete the form correctly could result in a delay to the review of the operations manual and the issuance of any associated approvals. Further guidance on completion of the form can be found on Page 4.

Uploading documents to the portal may be limited by file size. For larger documents such as manuals please follow the following steps:
- Submit your NPA via the portal to receive your unique AFSS tracking reference number.
- Email supporting files to npa@caa.co.uk with your AFSS reference in the subject line (as a zip email attachment if necessary).
- Alternatively, send via SharePoint link.
Please forward all published documents to the NPA mailbox, copying your inspector.
