Certificate of Training or Experience for Grant of Flight Radiotelephony Operator’s Licence

Form to allow provision of declaration of training or experience for grant of an FRTOL as an upload to the e-licensing system. This form is intended for use in the provision of evidence in support of an application made to the CAA using the CAA's e-licensing system. Once completed the form should be scanned or photographed and uploaded by the applicant as part of an online application to the CAA.
