Model Aircraft Exemptions - Application - Over 20 kg

This form should be used to apply for an exemption to test fly or fly a model aircraft which has a mass over 20 kg, to renew such an exemption or to add a pilot to an existing exemption. CAP 658 should be read before submitting the form.
