Documents and analyses the findings of a 1995 review of accidents. Terms of reference were to undertake a review of serious and fatal GA accidents focusing on causal factors with the aim of reducing their number and also to produce a report with recommendations to the CAA's Safety Regulation Group. This was a follow up to the findings published in 1988 as CAP 542.
