This is a reconstruction of an actual incident where a helicopter malfunction forced the pilot to divert to an unfamiliar airfield. Autopilot mode confusion during the glidescope capture results in loss of control and breakdown in collective situation awareness.
Decision making and review, automation, communication, monitoring and intervention are all identified as contributory factors. These issues are discussed by Bristow Group's Director of Aviation Safety.
